Terms of Service

1. Acceptance of terms

By downloading, installing, or using TRT Companion ("the App"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use the App. The App is developed and operated by Made Form Ltd ("we", "us", "our").

2. Not medical advice

TRT Companion is a personal health tracking tool. It is not a medical device, does not provide medical advice, and is not intended to diagnose, treat, cure, or prevent any medical condition. Nothing in the App constitutes or replaces professional medical guidance from a qualified healthcare provider.

The level estimation chart uses pharmacokinetic models based on published half-life data. It produces an estimate only and does not represent actual blood levels. Do not adjust your treatment based on this estimate without consulting your prescribing physician.

3. Personal use only

The App is intended for personal health tracking only. Do not use it to make clinical decisions. Always consult your prescribing physician or endocrinologist before making any changes to your testosterone replacement therapy protocol.

4. AI blood test import

The AI blood test import feature uses artificial intelligence to extract values from lab report images and PDFs. While we aim for high accuracy, AI extraction is not guaranteed to be error-free. You must review all extracted values before saving them to the App. You are responsible for verifying the accuracy of any imported data.

5. HealthKit

The App integrates with Apple HealthKit to read health metrics including sleep, heart rate, heart rate variability, workouts, and body composition. This data is used solely to enrich your personal health tracking within the App. HealthKit data is never sent off your device. You may revoke this permission at any time in your device Settings.

6. Subscriptions

Access to TRT Companion is provided via an auto-renewing subscription, managed entirely by Apple through the App Store. Subscription prices are displayed in the App Store in your local currency. By subscribing, you also agree to Apple's media services terms of sale. The following terms apply to your subscription:

• Payment is charged to your Apple ID account at confirmation of purchase, or at the end of any free trial period.
• Subscriptions automatically renew unless cancelled at least 24 hours before the end of the current billing period.
• Annual plans include a 7-day free trial. If you do not cancel before the trial ends, payment is charged automatically.
• You can manage or cancel your subscription in Settings > Apple ID > Subscriptions on your device.
• Refund requests are handled by Apple in accordance with their policies and applicable law in your jurisdiction.

7. Your statutory rights

These Terms do not affect any rights you have under the mandatory consumer protection law applicable in your country or region. Regardless of what these Terms say, you retain all rights provided to you by law, including any right to a remedy where a service is not as described or not fit for purpose.

8. Limitation of liability

Nothing in these Terms limits or excludes our liability for: death or personal injury caused by our negligence; fraud or fraudulent misrepresentation; or any other liability that cannot be limited or excluded under applicable law in your jurisdiction.

Subject to the above, the App is provided "as is". To the fullest extent permitted by applicable law, we exclude all implied warranties, conditions, and representations. Our total aggregate liability to you shall not exceed the amount you paid for your current subscription period.

In particular, we are not liable for: health or treatment decisions made based on data displayed in the App; errors in AI-extracted blood test values; pharmacokinetic estimates produced by the level estimation chart; or correlation analyses presented in the App. You use these features at your own discretion.

9. Data and privacy

Your use of the App is governed by our Privacy Policy, which forms part of these Terms. Your health data is stored locally on your device and optionally backed up via iCloud. We do not operate servers that store your health data.

10. Intellectual property

The App, its design, code, and content are the intellectual property of Made Form Ltd. You are granted a limited, non-exclusive, non-transferable, revocable licence to use the App for personal, non-commercial purposes in accordance with these Terms. You may not copy, modify, distribute, sublicense, or reverse-engineer any part of the App.

11. Changes to terms

We may update these Terms from time to time. We will notify you of material changes within the App. Continued use of the App after changes take effect constitutes your acceptance of the updated Terms. We will always indicate the date of the most recent revision at the top of this page.

12. Governing law

These Terms are governed by the laws of England and Wales. Any dispute arising out of or in connection with these Terms or your use of the App shall be subject to the non-exclusive jurisdiction of the courts of England and Wales. Nothing in this clause affects any right you may have to bring or defend proceedings before the courts of your own country under applicable mandatory consumer protection law.
